Introduction

Before you start your quest, take your diving gear, nails (30 or more recommended), hammer and four regular planks. Start the quest by talking to Brother Tranquility, in Mos Le'Harmless docks. He tells you he and his fellow monks are in trouble, and asks for your help. Once you agree to assist him, he will teleport you to Harmony Island.

Go out of the monks' hiding place, and walk a bit north until you see a statue. Pull it to the side, and walk downstairs. The tunnel is full water, so you'll have to put your diving gear on. Run till the end of it, and repair the stairs. Once you're done, climb all the way up.

Curing the Monks

Look through the peephole. Next, go back to Brother Tranquility (through the water tunnel) and tell him what you saw. He will suggest removing the poison that causes the monks' disease. Travel to the monastery west of Edgeville, and search the bookcase in the south-west part of it. Once you aquired the book, go to Edgville's bank and take a holy symbol. Now travel back to the monks' hiding place, put on your holy symbol and left click the book. This will cure the monks.

Now that the monks are cured, they would like to receive their brains back. Unfortunately you can't exactly help them in that matter, so you'll have to visit an old friend of yours, Dr. Fenkestrain. Travel to the doctor's castle, you will find him in the top floor. The doc agrees to help, however he's allergic to teleportations, therefore you'll have to find another way.

Filling the Crate

Head to Canifis, and take your Ring of Charos (a) from the bank. Make sure you wear it, and go talk to Rufus, the meat store owner. He will agree to let you use a crate, but you'll have to fill it yourself, and he'll check if it's good. Rufus think you're planning on transporting cats, so you will have to create 10 wooden cats to trick him. If you have a workshop in your house, go to it. If not, you can use a friend's house. Before you go, take 10 normal planks and 10 furs. Once you're there, craft 10 wooden cats.

Take with you 4 planks, 50 or more nails, and a hammer. Travel back to the doctor, where there is now a spot to build the crate on. Build the crate, and afterwards build a falsebottom to it. Finally, add the wooden cats to it and blow the wolf whistle. Rufus will come, and after a quick authentication he will give you a shipping order. Use it on the crate, and it will be teleported to the island.

The Final Fight

Travel back to Harmony Island, but stop at the bank on the way and take your diving gear, a hammer, and gear up to fight 5 sorebones, level 70 each. When you reach the Harmony Island, climb down the ladder in the monks' hiding place and talk to Doctor Fenkestrain. He will tell you he needs some tools for the surgery. Go outside, and run north until you reach an abandoned ship. In the first floor, there is some fuse in the locker, western part of the ship. Take it, and then go upstairs and take a tinderbox, and some gunpowder. Go to the main building in the island, put the fuse and the gunpoweder on the door, and when you're ready, light the fuse with your tinderbox to blow the doors off!

Kill all the sorebones, until you get all the required items for the surgery:

  • 3 Bell jars
  • A pair of Brain tongs
  • A Cranial Clamp
  • 30 Skull staples
  • A hammer (the one you brought)

Once you're done, head back to the doctor and give him the tools. A short cover up of a cat playing with a ball of wool will show up, to censor the surgery.

Once the surgery is complete, head back to the building with the sorebones. The battle with Barrelchest will now begin. Barrelchest is a fairly easy boss, and high level players can beat him using super potions and 3 rocktails. Using a crush weapon is very recommended, as Barrelchest has a weakness for it. Godswords, Zamorakian spear, and other crush weapons are good. Barrelchest will disable your prayer and lower your pray by 5 every hit, but turning on protect from melee every time does help against the next hit.

Pick up the barrelchest anchor, and go back to Brother Tranquality and inform him about what happened. Note: you can fix the barrelchest by paying 250K to Smith the Pirate on the docks of Mos Le'Harmless.
